New Megajolt V4 Rev D

We've made some changes to the current Megajolt system- hence the Revision 'D' moniker.

Functionally it's exactly identical to the current Revision 'C' board- software/firmware/connectors; The changes centered around a new extruded enclosure (you may have seen earlier) and move to all surface mount parts.
